An employee was trying to unjam two steel bars on a conveyor when the photo sensor sensed movement and began to cycle, causing the employee's right hand to be dragged into the conveyor. The employee suffered a fractured right hand and was hospitalized overnight. The machine was not locked out at the time.
An employee was using an oxygen torch to open a tap hole on a ladle. As the employee penetrated the tap hole with the torch, molten steel burned the employee. The employee was hospitalized with second- and third-degree burns to the left hand.
