Final narrative
On January 28, 2019, at approximately 1:30 p.m., the injured employee, a scrap operator and employee in training for locomotive operations, was performing scrap railcar spotting operations with a trainer at the half track at Timken Steel Corporation scrap yard. Both employees were riding the leading edge of the railcars proceeding in an easterly direction. The injured employee was riding on the north side of the railcar and the operator was on the south. As the railcar went around a slight bend in the tracks, the injured employee was struck in the lower left leg by a piece of scrap steel. The scrap steel was frozen into the ground and sticking up and was not clear of the railway. The injured employee suffered a broken tibia and fibula that required surgery to repair.

Both employees were in full body harnesses and were connected by a self-retracting lifeline (SRL) to a rail fall safety system. The train started moving north and the employees realized that the cars were going to move beyond the fall protection coverage and decided to unhook from the rail fall protection. Employee 1 assisted the injured employee and unhooked from the system. As the injured employee attempted to unhook, the SRL ran out of cable, roughly 25 feet from the rail shed. The injured employee was pulled onto his back and remained stationary on his back, held by the SRL cable, as the rail cars moved under him. The employee sustained a laceration and contusion to the top of the head; bruising and scrapes to the lower back, legs and arms; a cut on the right hand; and a bruised thumb.

An employee was working to close a furnace door. The hydraulic mechanism that opens and closes the furnace door was non-operational and had been disconnected to allow the furnace door to be opened manually. As the employee was releasing the pins that held the door open, the door and the hydraulic cylinder fell, pinching their left hand between the hydraulic cylinder's base plate and the furnace. The employee suffered amputation of the little fingertip.

A rolling mill was shut down due to a cobble between two mill stands. The injured employee was cleaning the cobble from the delivery side of stand A, while another employee went to the control panel to move stand B and stand A moved instead. The injured employee's right thumb got caught between the delivery guide of stand A and the looper trough. The tip of his thumb required surgical amputation to the bone to allow for stitches.
