At about 4:30 PM on 09/06/2016, an employee operating an overhead gantry crane was using a handheld remote and a C-hook to remove a specific coil of slit steel from among other coils in storage. He became caught between the loaded C-hook and another fixed coil in storage, fracturing his left clavicle (collarbone) and two ribs.

An employee was carrying a load of steel coils with a crane. The load he was carrying hit a stack of coils in front of him. The coils fell on the employee, knocking him and the coils behind him to the ground.

Employee was cutting bands from a group of slit coils. The coils were on the turnstile arm. They were not in a static position. When the last piece was cut, it fell off the turnstile and hit the employee. The piece was approximately 60 inches long and weighed 2000 pounds.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.

An employee was removing a roll (tooling) from a machine when their left little finger was pinched between the roll and the machine, resulting in a partial amputation.

An employee was rolling a piece of steel in the belt of a shape roller machine when his finger was pinched between two pieces of steel. His right ring and middle fingers were amputated.

An employee was cutting a length of steel coil when he noticed the steel might have cross-break defects. He used his gloved hand to feel for and clean the defected spots when his glove got caught in the measuring wheel, which rolls along the top of the steel to ensure accurate cuts. The employee's arm was pulled down into the machine until his body was stopped by a support bar that goes across the machine. The employee's arm was fractured at the elbow and required hospitalization.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
