Employee #1 was operating a stand-up forklift with forks trailing, headed south on a dock area.
Employee #2 was also operating a stand-up forklift with forks trailing in the direction of employee #1. Employee #1 stopped his forklift to activate a door's pull cord to enter a freezer when his forklift was struck by the forklift of employee #2.
The back-to-back impact caused Employee #2's left foot to become caught between both forklifts, fracturing the big toe and partially amputating the fourth toe.

An employee was riding on a powered pallet jack near a sewer drain. The pallet jack hit a dip near the sewer drain, and the employee s left foot hit a clamp truck parked in a charging station. He suffered a cut to the big toe, as well as fractures in the toe and the foot.
