An employee servicing a wood chipper suffered a partial am[utation of the right ring finger after going under the machine to engage the belt. The chipper was not locked out/tagged out at the time.
An employee was mowing the lawn of a commercial property when he saw an obstruction on his path. He picked up a piece of rebar steel from the ground, placed the rebar on the mower's floor deck, and continued mowing the lawn. Due to the mower's movement and vibration, the piece of rebar fell and struck the tire of the mower, then deflected back and hit the employee's shin, causing a minor laceration and contusion on his leg. He was hospitalized and required surgery.
