Final narrative
An employee was resizing parts in a press machine and amputated the finger print portion of his left middle finger. Lockout/tagout controls were not in place at the time of the incident.

When the part was loosened, the actuators engaged and clamped down on his left hand amputating the tip of his left middle finger.
