Final narrative
An employee was working a welding machine and a part got stuck. The employee reached in with his right hand to loosen the part and braced himself on the machine with his left hand. When the part was loosened, the actuators engaged and clamped down on his left hand amputating the tip of his left middle finger.
An employee was resizing parts in a press machine and amputated the finger print portion of his left middle finger. Lockout/tagout controls were not in place at the time of the incident.
