On 10/13/2016 at approximately 5:30 am, an employee was cleaning a film tear out of a film line MDO roll section when his right hand got caught in the film line MDO roll section and was crushed. The employee was hospitalized and the right hand was amputated the next day.

An employee was removing woven fabric from a coater machine using scissors. The rollers caught the end of the scissors and twisted them and the employee's fingers, resulting in a fracture and lacerations. The employee was hospitalized and underwent surgery to their little finger.

An employee was trying to guide a shaft (on a hoist) into the primary arms on a laminator. The clamps on the laminator were activated and closed on the employee's right ring finger, causing an amputation to the fingertip.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

At 12:45 a.m. on September 21, 2025, an employee was working to clear an obstruction in a machine when his left hand came into contact with a turning screw inside the machine. His index and middle fingers were amputated.

An employee was performing maintenance on a sheeter. He had pried up a steel access plate and was lifting it when it slipped and fell on his left middle finger. The finger was pinched against the floor, and part of the fingernail and fingertip was amputated.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
