An employee was using a rag to clean ink blots from the rollers of a flexo press. The rag became caught in the rollers and pulled the employee's left hand into the machine's rollers. The employee sustained amputation of the left index and middle fingertips.

An employee was removing woven fabric from a coater machine using scissors. The rollers caught the end of the scissors and twisted them and the employee's fingers, resulting in a fracture and lacerations. The employee was hospitalized and underwent surgery to their little finger.

An employee was trying to guide a shaft (on a hoist) into the primary arms on a laminator. The clamps on the laminator were activated and closed on the employee's right ring finger, causing an amputation to the fingertip.

An employee was working with a 12-foot coater machine. While changing a roll, the employee was caught in the material on the roll and pulled into the coater. The employee suffered a laceration to the head/face and was hospitalized.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was clearing a clog in the glue gun machine. When the clog cleared, his left index finger struck the fan blade, resulting in medical amputation.

An employee was operating a clamp truck to transport material to a workstation from the rear of the warehouse. The forklift was traveling near the rear baler area when another employee stepped into its path and the front right tire ran over his left foot. Their left foot and toes were crushed resulting in a dislocation and broken bones, as well as a laceration to the foot.

An employee was grinding a burr off a piece of stainless steel using a 12-inch stationary disc sander when his right thumb contacted the sanding disc, resulting in amputation at the first knuckle.

On February 16, 2023, an employee was removing a piece of film from a machine when his right thumb was pulled into the nip-point and amputated.

On December 16, 2022, an employee was feeding a book through a machine and guiding the wire when their left index finger was amputated at the first knuckle.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

Sargento Foods Inc

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

Asher Enterprises, Inc.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
