The employee performed lockout/tagout on the unit but the fan continued to rotate because of the positive pressure in the building. The employee was attempting to slow the rotation of the fan by slowing the drive belt when the employee's right hand was pulled into the drive pulley, amputating the right middle fingertip.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.
