An employee was climbing a ladder and fell fracturing a hip.
An employee was performing maintenance on an exhaust fan motor for a salon. The employee performed lockout/tagout on the unit but the fan continued to rotate because of the positive pressure in the building. The employee was attempting to slow the rotation of the fan by slowing the drive belt when the employee's right hand was pulled into the drive pulley, amputating the right middle fingertip.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
