Final narrative
Two employees were changing out a belt on a piece of equipment. They locked out the machine and were pulling belts on a pulley. The first employee was aligning the belt around the next groove when the second employee manually gave the shaft another turn, trapping the first employee's finger under the belt and above the pulley. His left index fingertip was amputated.
The injured employee was cleaning parts inside a machine that makes boxes. She went to grab a piece of material when while the chain and sprocket was being manually moved and her left index finger was caught in the mechanism. The employee's fingertip was amputated with bone loss. The machine was locked out/tagged out at the time.
