Prairie Farms East Side Jersey Dairy

An employee was riding on a powered pallet jack near a sewer drain. The pallet jack hit a dip near the sewer drain, and the employee s left foot hit a clamp truck parked in a charging station. He suffered a cut to the big toe, as well as fractures in the toe and the foot.

An employee went to step off material handling equipment to grab their jacket when the throttle of the vehicle engaged. The vehicle reversed and the employee's left foot was crushed between two material handlers, resulting in a fracture.

PRODUCTIV INC

An employee was working at a height of approximately 20 feet and was standing outside his forklift's basket attachment. His forklift was struck by another forklift, causing the employee to fall to the warehouse floor below. The employee sustained facial injuries, broke two ribs and an arm, and also suffered a head injury. The employee was wearing a harness at the time but it was not connected to an anchor point.

Del Monte Fresh Produce N.A., Inc.

On November 8, 2023, an employee was operating a stand-up pallet jack in the warehouse. As he was backing up, the pallet jack struck the basket of a parked aerial lift. His right ankle became caught between the pallet jack and the aerial lift basket resulting in a fractured ankle.

John A. Biewer Co. of Wisconsin

On October 26, 2023, at approximately 12:00 PM, an employee operating a four wheeler was on their way to an off-property break location. The employee stopped the four wheeler near the administrative office to allow a log truck to exit the property. The log truck backed up, and the employee's left ankle was caught between the log truck trailer and the four wheeler, resulting in a fracture and laceration.

Thermal Concepts, LLC

An employee was working next to a 12-inch, 21-foot-long pipe that was being lowered using a 1-ton chain fall. The chain fall slipped, causing the steel pipe to drop. It struck his left hand, causing the partial amputation of the ring fingertip and the loss of the fingernail.

TRUE MANUFACTURING

Three employees were reorganizing a work area by staging material-handling tables. Two employees were pushing a table while the third employee guided it. The table became stuck on the wheel of another nearby table and while the two employees tried to push it free, the third employee moved the other table. The newly freed table moved forward into the other table, and the third employee's right middle finger was caught between the two. A third of the fingertip was amputated.

True Manufacturing Company

An employee was fabricating an evaporator fixture. The fixture was sitting on a table and the employee was utilizing a hoist and chain sling hooks to elevate one side of the fixture into position to weld a piece on it. The hoist moved slightly on the bridge trolley, creating slack in the chain that caused one of the legs of the sling to come out of the square tubing. The evaporator fixture came down onto the employee's right ring finger, pinning it against the table. The employee's finger was partially amputated and was later surgically amputated above the knuckle.

Trane

An employee was operating a shear when its blade came down and amputated his left middle and ring fingertips. The machine was guarded at the time.

Johnson Thermal Systems, Inc.

An employee was removing material from a pallet rack using a standup forklift when an upper shelf was knocked out of place causing a pallet of industrial fans to fall 12-15 feet. The employee was struck by the falling pallet. The employee sustained a laceration to the front-right of their head and fractures to three vertebrae.

ABC Supply Company

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

Burke Builders Inc

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

Circle Graphics, Inc.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Takkion OPS Management

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
