An employee was conducting an inspection onboard a moored vessel. While exiting a space, the employee stepped onto an unmarked, unsecured deck plate covering a void. This caused the plate to shift and the employee fell into the space below.
On 11/22/2023, at 8:35 AM, an employee was stepping onto a barge when they slipped on a wet surface and fell on their knee. The employee sustained a torn ligament in their knee requiring hospitalization and surgery.
