An employee was setting culverts along a highway. After a culvert was in place, the employee slipped on the embankment. The employee grabbed the rim of the culvert to catch his fall and lacerated three fingers on the left hand.
An employee was on a haul truck trailer helping a mechanic tighten bolts. When they were finished and coming off of the trailer, the employee slipped on mud and fell to the ground, breaking an arm.
An employee was between the trough and casting pit, cleaning a filter box at the end of a cast.
