Two employees were putting another section of boom on a crawler crane. One end of the boom was resting on the ground and the opposite end was elevated approximately 2.5 feet in the air. An employee was under the elevated section of the boom, removing pins that would allow the boom to be separated. When the second of the two pins holding that section was removed, the crane boom fell on the employee. The employee sustained fractures.

An employee was working on a precast riser form, trying to adjust a steel plate at the top of the form. When a final bolt was loosened, the plate dropped and crushed the tip of the employee's left little finger. The fingertip was partially amputated.

An employee was standing on a precast bed instructing a team member to strike a buttress to straighten the stressed strand in the form. The buttress spun due to the impact from the hammer and pressure from the strand. The pressure from the strand then pulled the buttress through the opening. The strand struck the employee's foot, knocking him off his feet and landing on his shoulder. The employee sustained an ankle injury and a dislocated left shoulder.

An employee was driving a truck, pulling away from a plant, when six vape pen batteries in his right front pants pocket ignited. He suffered severe burns to the upper right leg.

An employee was sanding the rough edges of a concrete wall while standing on a yard trailer platform when the employee tripped and fell approximately 4.5 feet to the ground, suffering a head injury and requiring hospitalization.

An employee was walking to their truck to look for a lid. When they stepped between the truck and a dock plate, they sustained a knee injury that required surgery.

An employee was standing on plant bed timbers, using a powered hand trimmer to trim the shrubs in the plant bed. The timber gave way, breaking into several pieces. The employee lost balance and fell with the trimmer in his hand. He suffered a severe laceration to the right wrist and hand.

A mail carrier slipped on ice and sustained a strain that required hospitalization.

An employee was loading phosphoric acid onto a rail car and preparing to secure the rail car for transport. While repositioning the loadout spout to secure it back to the drip pan holster on the dock, the spout contacted the handrail of the rail car, causing the employee to lose balance. The employee stepped back and their right foot entered the opening for the rail car, causing him to twist/pivot on his left leg. The employee's lower left leg was fractured.

On November 14, 2023, an employee was receiving a delivery from a truck when some of the merchandise fell from the truck s lift and struck him. He then fell to the ground and his head struck the sidewalk, resulting in an injury that required hospitalization.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was patching precast concrete. He fell from a concrete wall support to the ground due to wind. He was hospitalized with fractures to his left ankle and right knee.

An employee was opening a precast concrete form and rolled his ankle. The employee required surgery on his ankle.

A driver slipped and fell to the ground while exiting his truck. He sustained a lacerated spleen.

An employee was cutting wood on a table saw when the wood got jammed. While clearing the jam, the wood moved forward and his left index and middle fingertips were amputated.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
