Final narrative
On December 29, 2016, at approximately 11:30 a.m., a crew was moving skids out of the bottom of a large excavation. The skids had been placed in an excavator's hoe bucket on the ground. An employee was unloading the skids from the bucket and handing them to the injured employee, who was positioned next to the track hoe bucket and a super sack of sandbags. At this time, another employee walked by the hoe operator and asked him to take over the track hoe. As he twisted around to speak, his safety vest caught the operating lever and caused the bucket of the track hoe to move and pinch the injured employee's left ankle between the hoe bucket and super sand bag. His lower left leg was fractured just above the ankle.
