An employee was operating a mixer in the production department and was forming a 15 inch round concrete pipe. The employee's right hand was caught inside the pipe between a concrete rotor and the form resulting in the amputation of three fingers.

Foley Products Company

An employee was starting the filling process for an 18-inch elliptical form and then prepared headers for the next step of production. The employee loaded the headers into the elliptical header form and rotated a lever to engage the lifting lugs when his right middle fingertip was pinched by the lever, resulting in amputation.

FOLEY PRODUCTS COMPANY

On March 13, 2018, at approximately 9:00 a.m., an employee was using a crane to unload a 6x6 concrete box when the crane became unbalanced and rolled over, pinning and crushing the employee's legs under the fuel tank. He was hospitalized.

An employee was anchored to a movable piece of equipment called a "lid" with a self-retracting lifeline harness. An overhead crane lifted the equipment, causing the employee to be pulled by his harness. The employee was pulled off the walkway and swung over the wall of the kiln. His body and head struck the wall. The employee lost consciousness and sustained a laceration to the side of his head that required stitches. The employee also sustained multiple fractured ribs and fractured vertebrae.

Forterra Pipe And Precast, LLC

An employee was using an overhead crane to lower a manhole cylinder to the ground to complete maintenance and cleaning. While removing a lifting hood from a manhole core (steel cylinder), the cylinder rolled and the employee's left little finger was caught between the crane hook and the lifting eye. The employee's little fingertip was amputated just below the nail.

An employee was moving a 4x4 box jacket with an overhead crane. The employee hooked a chain to the jacket and put tension on it. It spun awkwardly, striking the employee and knocking him over. The employee fell on the same level and sustained a fractured rib and lacerations to his lower left abdomen and backside.

Forterra Concrete Products, Inc.

An employee was guiding a forklift operator who was placing a 48-inch form jacket (open cylinder) over a core (solid, stationary cylinder). The employee was positioned on the ground adjacent to the core. As the suspended jacket was positioned over the core, a gust of wind caused the jacket to move. The employee reached to steady it and the jacket moved downward pinching his left little finger between the jacket and the core and resulting in a fingertip amputation.

Universal Precast, Inc

An employee was at the bell packer when the scraper of the bell packer struck his hand. The employee sustained broken fingers and degloving injuries involving their right ring and little fingers.

Howard Industries Inc.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

Hobby Lobby Stores, Inc.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

Owl's Head Alloys West Point

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

Ingalls Shipbuilding

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
