An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee was using a pallet jack to remove a pallet from the back of a trailer when the strap on the pallet broke. The momentum caused the employee to fall from the trailer to the ground, resulting in an L1 vertebra fracture. The employee was hospitalized.

On September 11, 2024, an employee was walking in the trailer and tripped over plastic strapping on the floor. The employee fell and broke their right hip.

An employee was exiting a trailer when their foot slipped, causing them to fall to the ground and suffer a broken left leg.

An employee was operating an electric forklift when the forklift slid off of the dock while making a left turn and hit the ground. The employee fractured his right arm, right-side ribs, and collarbone; suffered a bruised liver; and required staples on the right side of his head.

An employee was training on a jockey truck. Upon stepping out of the truck, the employee was struck by a rubber tire gantry crane, resulting in amputations of the right leg above the knee and left leg below the knee.

An employee was standing at the rear of a double pallet jack. Another double jack approached, and the employee's lower leg was caught between the two pieces of equipment. He suffered a broken tibia and fibula.

An employee was unloading large material bags on a loading dock. A forklift tine pinned the employee's foot against a pallet, causing the employee to fall and suffer a broken ankle.

An employee was pushing a container when they were struck by a forklift. The employee sustained a left leg fracture.

A driver was loading a trailer. The driver fell out the back of the trailer and fractured his left elbow on the ground. The employee was hospitalized.

An employee was picking bulk bags for shipments. While cleaning pellets off the ground, a stacked bulk bag (full of plastic pellets) collapsed and fell on the employee. The employee sustained fractures to their ribs, pelvis, and left femur.

An employee was delivering goods to a customer. He was folding the truck's lift gate when his left middle finger was pinched in the lift gate, resulting in an amputation.

An employee was operating an electric pallet jack, delivering pallets of groceries to a store. The employee was walking backward when her foot was pinned between the pallet jack and a video pole, resulting in lacerations and fractures to her ankle and heel. She was hospitalized.

An employee was tightening a turnbuckle while attempting to conduct lash-back operations on a ship when their left gloved hand was caught between the upper and lower turnbuckles. The employee suffered a crush injury to the little finger that resulted in a fingertip amputation.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.
