An employee was training to operate an electric pallet jack (EPJ) to pick orders. The employee released the EPJ throttle and it started to slow. The employee then stepped off the EPJ while his hand was on the EPJ control handle. The EPJ continued its forward momentum and the employee's right foot was caught between a pallet in the aisle and the EPJ. The employee sustained a crush injury to his right hallux that required surgery and was hospitalized.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
