An employee was on a ladder tying off electrical wire on the ceiling when he suffered an electrical shock. He blacked out and fell off the ladder, hitting his head, twisting his ankle, and fracturing and contusing his back.

An employee was training to operate an electric pallet jack (EPJ) to pick orders. The employee released the EPJ throttle and it started to slow. The employee then stepped off the EPJ while his hand was on the EPJ control handle. The EPJ continued its forward momentum and the employee's right foot was caught between a pallet in the aisle and the EPJ. The employee sustained a crush injury to his right hallux that required surgery and was hospitalized.
