An employee was attempting to clean the focus eye lens in a trough that wood passes through on the lay-up assembly line. The machine started while the employee's right ring finger was in the strike area and a piece of wood crushed the finger, breaking it and causing the fingertip to burst.

An employee was changing over from running 3 1/8 to 5 1/8 beams at the sander on the finish line and placed a 2x6 board under the sander side head. The machine grabbed the board, pulling their left hand into the sander side head and causing a severe laceration on the left index finger and thumb. The index finger was amputated.

An employee was removing veneer from a composer machine using a pipe pole. The employee made contact with the blade in the machine, which partially amputated the employee's left thumb. The employee was hospitalized.

The injured employee was on the ground level receiving an OSB board from a co-worker who was on a 10-foot-high catwalk. The board was 4 feet by 25 feet and weighed approximately 270 pounds. As it was being slid down a handrail by a crew of three employees, the board went over the rail and landed on the injured employee's left foot, resulting in a fracture.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.
