Final narrative
A temporary employee was resting on top of the depucker machine guard housing while bottles passed by on the conveyor belt. She braced her left hand underneath the machine's conveyor belt guard to push herself back up to a standing position when her left hand made contact with the moving conveyor belt resulting in the amputation of the first joint of her left index and ring fingers. She was hospitalized.
A mechanic was assisting another employee to remove an accumulation of material from a machine. The mechanic shut down the machine and went to clear the material when his fingers were caught in the roller, resulting in amputations to his index, middle and ring fingers. The machine was not guarded at the time of the incident.
