A temporary employee was using a metal drum lift to lift a 55-gallon plastic drum to raise and tip the drum over and pour the suntan product mix into a container to be weighed. He placed his hand on the lift's clamp while the drum was in a vertical position. The drum slipped and his right index finger was caught between the top lip of the drum and the clamp resulting in the amputation of his right index fingertip.

A mechanic was assisting another employee to remove an accumulation of material from a machine. The mechanic shut down the machine and went to clear the material when his fingers were caught in the roller, resulting in amputations to his index, middle and ring fingers. The machine was not guarded at the time of the incident.

She braced her left hand underneath the machine's conveyor belt guard to push herself back up to a standing position when her left hand made contact with the moving conveyor belt resulting in the amputation of the first joint of her left index and ring fingers. She was hospitalized.

On 01/19/2017, at approximately 12:00 p.m., an employee was walking into the work site when she tripped and fell over the curb and into the doorframe. She caught the edge of the hinged door and then fell to the concrete floor, hitting her forehead and scalp, breaking her nose, and contusing her neck.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.
