Final narrative
A temporary employee was using a metal drum lift to lift a 55-gallon plastic drum to raise and tip the drum over and pour the suntan product mix into a container to be weighed. He placed his hand on the lift's clamp while the drum was in a vertical position. The drum slipped and his right index finger was caught between the top lip of the drum and the clamp resulting in the amputation of his right index fingertip.
A mechanic was assisting another employee to remove an accumulation of material from a machine. The mechanic shut down the machine and went to clear the material when his fingers were caught in the roller, resulting in amputations to his index, middle and ring fingers. The machine was not guarded at the time of the incident.
An employee was transferring a steel mold plate from one work bench to another using an overhead hoist when the mold plate already on the work table began to fall/tip over. As he tried to catch the plate, it crushed his right hand.
