An employee was feeding bar stock into a CNC Machine when the bar stock became jammed in the feeder. When he pushed the bar stock forward, it rammed into the side of the machine and struck his left middle finger, partially amputating it and avulsing a nailbed.

An employee was checking to see if a tester was clogged on the TRV Machine. The machine was not locked out/tagged out at the time. When he placed his hand to check the tester, the machine came down and cut his right ring fingertip.
