An employee was gassing up a furnace and attempting to light the pilot using a wand (igniting device). During a previous shift, one of the furnace's two gas valves was left open. When the employee began the start-up process, he opened the main gas valve without noticing that the other valve was already open. The front door on the furnace was open and a flash fire occurred, causing burns to his hands and forearms.

An employee was using a circular saw to cut flashing. He was finished with the cut and started walking away when his fall protection ropes became entangled in the saw and the saw was pulled into his leg. The employee sustained a left hamstring laceration.

The employees went to evacuate the area in anticipation of the wall panel falling over. An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.
