An employee was removing material from an egg conveyor system when her hand was pinched between two rollers, resulting in a fractured and dislocated right thumb.

On March 12, 2019, a Trillium Farms maintenance employee was cutting a T-shaped piece of plastic with a table saw. The blade amputated the employee's right index finger. The saw was unguarded at the time.

An employee was repairing belts on a motor drive when a coworker started the motor. The employee's glove was caught, pulling the employee's right pinky finger into a pinch point between the belt and pulley. The employee's right pinky finger was amputated to the first knuckle.

An employee was transferring young chickens from a cage onto a cart. The employee lost balance and slipped from the access step falling backwards about 22 inches against a metal railing. The employee was hospitalized for a rib injury and possible pneumothorax.

An employee was attempting to hitch a box trailer to a pickup truck. As the employee was aligning the ball with the trailer hitch, their fingers were inside the hitch. When the ball aligned with the hitch the fingers were pinched resulting in amputation of the left middle fingertip. The fingertip was reattached at a medical treatment facility.

On December 19, 2023, an employee was setting up a copper wire drawing machine when his left ring finger was pinched between a copper wire and a machine part. The employee's finger was partially amputated.

An employee was examining a machine when a part moved up and amputated a fingertip on their left hand.

An employee was checking the gusset seal on a machine. He moved to the next cross sealer and the machine activated, causing an amputation to a left-hand finger. The employee was hospitalized.

An employee was evaluating a coiling machine to verify that the blade was present inside it. The machine's weld function was activated, and the machine crushed the employee's fingers, resulting in the amputation of the left middle and ring fingers.

An employee's right hand was caught in a wire drawing machine, resulting in amputation.

Three employees were transferring a metal lid using a manual chain hoist to place it onto a cart. While they were adjusting its position, the lid shifted and the injured employee's left hand became caught between the hoist hook and the handle, resulting in a laceration and amputation of their ring fingertip including the tuft.

An employee was using an 8-foot metal pipe to remove a manure blockage at the transfer to an incline conveyer. The employee s left hand was placed on top of the pipe while removing the blockage. The pipe pushed their left hand into the wooden railing of the platform they were standing on, catching their thumb in a pinch point and resulting in partial amputation.

An employee was driving a farm truck with the dump box raised when he went through a farm building accessway and the dump box struck the entryway of the building. The truck lifted up and came back down, jolting the employee. The employee sustained a back injury.

An employee was doing housekeeping work inside a chicken house. The employee missed a step onto a walkway near the exit, fell into an 8-foot-deep pit, and landed on its concrete floor. The employee suffered a leg fracture above the ankle, as well as a chin laceration.

An employee was troubleshooting a manure belt to figure out why it kept turning off. The employee's right hand was pulled into the drive and compression roller of the long belt in the manure belt system, resulting in numerous injuries to their hand and forearm, including a partial amputation of the middle finger and a degloved palm.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
