An employee was walking on the grating when he slipped and fell. The grating gave way, causing the employee to fall an additional five feet. The employee landed on his legs fracturing the right leg in three places.

An employee was troubleshooting a strapping machine and spraying WD-40 into it when his hand felt like it was going to sleep. He rested his hand on the machine, and his ring and middle fingers were placed in a pinch point. He fractured two bones just below the nailbed, requiring surgery.
