Final narrative
On 3/17/17, at 3:00 p.m., an employee was inside a box truck loading vertical wooden panels when the panels fell against him, breaking his left femur which required surgery and hospitalization.
An employee was waiting for a powered industrial truck (PIT) to load a double door for transport. The PIT struck a storage rack that was holding 14 double doors; five doors fell onto the employee, resulting in a broken left hip (femur).
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.
An employee was unloading freight from a truck when a stack of doors fell over and knocked the employee to the ground. The employee sustained a fractured femur.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.
An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
