Final narrative
An employee was operating a slitter machine which unrolled steel coils. The coils develop slack as they are unrolled and to counter the slackness the operator manually inserts cardboard slats. As the employee inserted a slat, a gloved right hand was pulled into the coil. The middle, ring and little fingers were amputated. The thumb tip was also amputated.

An employee was threading a flat bar machine when slack got into the coil line. As the employee reached to fix the coil, his gloved left hand got pulled into the metal flat bar coil. The employee's left index fingertip was crushed in the machine, resulting in medical amputation of the fingertip including the loss of bone.
