An employee was threading a flat bar machine when slack got into the coil line. As the employee reached to fix the coil, his gloved left hand got pulled into the metal flat bar coil. The employee's left index fingertip was crushed in the machine, resulting in medical amputation of the fingertip including the loss of bone.

An employee was using a circular saw to cut smaller pieces of wood from 2- by 3-foot wood parts. The employee was holding the wood with his gloved left hand when the saw caught his glove and pulled his hand into the blade, amputating his left thumb tip and index fingertip.
An employee was operating a slitter machine which unrolled steel coils. The coils develop slack as they are unrolled and to counter the slackness the operator manually inserts cardboard slats.
