On July 31, 2023, an employee was getting up from his desk, when he tripped over a garbage can and fell to the floor. The employee sustained an injury to his shoulder that required surgery and was hospitalized.

An employee was in a loading dock, staging and sorting auto parts. He slipped or tripped on tailgate liners that had fallen to the concrete floor, striking his head against the floor.

An employee was running wire through a crawl space when he fell through the drywall ceiling and landed 18 feet below on a concrete surface, suffering blunt force trauma to the head, a severe laceration to the leg, and a fractured lower back.

An employee was lifting a motor and reaching to place it on a machine when they sustained a back injury. The employee was hospitalized.

On 12/22/2023, an employee was carrying bins filled with pasta to a refrigerator. As he lifted a 20-pound bin, he felt pain in his lower back. The employee sustained a herniated disc.

An employee was lifting a patient when they pulled a back muscle. She suffered back pain and spasms radiating into her leg with weakness and numbness. The employee's L3-L4 disc was herniated and she was hospitalized.

An employee was lifting boxes when they suffered a strained back, resulting in hospitalization.

An employee was lifting a bundle of insulation with a colleague when he felt severe pain in the right side of his lower back/hip region. The employee sustained a severe muscle injury.

An employee was descending a ladder when their foot caught on the edge of the bottom step. The employee fell and struck the floor, landing on their left hip and suffering a hip fracture that required hospitalization and surgery.

When the employee pulled it through, the cart fell over onto him and he fell to the floor, landing on his right hip. The employee's hip was fractured.

An employee was found on the floor within the enclosure of a laser machine. The employee was adjusting a sheet of metal on the machine while carrying a remote control, which actuated the machine, moving the carriage and causing the employee to be caught between the carriage and the machine table. The employee was hospitalized with a broken right shoulder and left ankle, and lacerations on their legs and ankles.

An employee was wrapping pallets using a plastic wrap dispenser when he felt a pop in his back along with a burning and stabbing sensation. The employee sustained the herniation of discs L4 and L5.

An employee was working at the end of a ramp when a powered industrial truck rolled down the ramp and broke the employee's foot.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
