On September 6, 2025, an employee was using a broom to sweep debris from the flatbed of a truck. The broom broke and the employee slipped and fell, resulting in a leg fracture.
An employee was cutting bands off a bundle of steel tubing. When he cut the final band, one of the tubes fell over, hit his leg, and landed on his foot causing a fracture.
At approximately 12:55 a.m., an employee was helping another employee load steel beams onto a tractor trailer. He picked up a beam with an overhead crane using the crane's remote control box. After attempting to maneuver the beam into an open space on the trailer, he determined the beam would not fit because it was too wide. He placed the beam on top of a smaller pile of beams. During this process, the chain holding the load caught the flange of an adjacent beam, tipping both beams and a tube. One of the beams landed on him. He was hospitalized with a fracture to his right ankle, a laceration on his right leg, and a sprain to his left ankle.
On November 8, 2016, an employee was cutting and removing bandings from bundles of tubing when two pieces of tubing fell and fractured the employee's right foot.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.
A load of steel angles was being lowered when the lifting chain hit the load, causing it to slide toward an employee. An angle slid and pinched the employee, catching both of their legs between the beams and an angle. The employee suffered two fractured legs.
An employee was on the deck of a trailer helping a customer unload tubes. The employee was working to place a cargo strap on the remaining tubes using a winch bar when it slipped off the tubing, causing him to lose his balance and fall headfirst onto the asphalt lot below. The employee suffered a contusion on his forehead, resulting in hospitalization.
An employee and a customer were unloading two rectangular tubes. The tubes were wrapped together. The plastic wrap was cut so that one tube could be picked up. As the employee was lifting the top tube, the other tube (still connected to the top tube) came down onto his hand and broke two of his fingers. He was hospitalized.
An employee was assisting as a forklift got pipe from a pipe rack. The employee was on one of the forklift's forks as it was raised to the top of the rack, a height of about 15 feet. He fell to the ground and suffered a dislocated and fractured right elbow.
On September 23, 2025, at approximately 7:43 p.m., an employee was using a knife to cut poly banding from a metal coil. The knife slipped, causing a laceration to the inside of their left arm at the elbow joint. The employee was hospitalized and required surgery to repair a damaged vein.
An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.
During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.
An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
