An employee was power-washing the rig. He shut it off to address an issue with some equipment and returned to relieve the pressure in the washer. He had his hand out when residual pressure in the washer line caused the water to release and cut through his glove and into his right hand, requiring hospitalization.

The chain severely punctured his leg and fractured his tibia.
