An employee was operating a scissor lift, traveling down a ramp in a parking garage. The lift started slipping due to the flooring conditions and a 9-degree grade. The scissor lift slid until it hit a curb at the bottom of the parking garage. When the lift hit the curb it stopped and the employee was ejected onto the concrete floor. The employee sustained 4-5 broken ribs, a fractured tail bone, a contusion to the right side of his head, and a lacerated liver.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
