An employee was standing on a truck step, inspecting the bays before making deliveries for the day. When he finished looking in the bay, he grabbed the strap of the door with his left hand and dismounted the step while his right hand was on the truck deck. The bay door closed on his right hand, partially amputating his middle fingertip, including the entire nail.

Pepsi Bottling Company

Two employees were scrapping an out-of-service forklift to send it to the scrap yard. As they were lifting the broken forklift with another forklift, a chain snapped and struck the employee in the chest/abdomen. The employee was knocked to the ground and sustained a lacerated pancreas and injures to the spleen and gallbladder. The employee was hospitalized and underwent surgery.

Matthews International Corporation

An employee was fixing a mechanical casket lift in a vehicle when the lift came down on their left-hand fingers, resulting in amputation of two fingers.

An employee was helping to remove the clamp attachment paddles from a forklift so they could then repair a hydraulic leak. The clamp fell and pinned the employee's right ring finger against the mast. The employee's finger was partially amputated.
