On May 15, 2017, an employee was attempting to change hoses on machinery when his fingers were caught and amputated in an unguarded moving chain and sprocket.
An employee was removing scrap from die area of a press when the die halves came together and caught the employee's left hand, resulting in crushing injuries. The employee sustained a transverse fracture and soft tissue injuries to the second, third, and fourth fingers.
