An employee suffered an amputation as a result of a workplace incident.
On September 25, 2023, an employee was working in the body department installing doors, hoods, and tailgates. The employee finished their shift and was hospitalized later that evening for a back injury.
An employee was operating a hydraulic machine to bend hollow aluminum approximately 1 inch in diameter when the machine pinched his finger. The employee sustained a right index fingertip amputation.
On November 19, 2020, a maintenance employee was assisting other employees to move equipment from one company location to another. The employees loaded several pieces of fencing/guardrails and a welding machine onto the forks of a forklift. The employees then backed the welder into the trailer and drove several blocks away. Upon reaching the facility, the employees parked the trailer at a dock that had an approximate 15 degree incline. The injured employee went into the trailer to retrieve the equipment and found the fencing had shifted between the welder and the forklift. As the employee attempted to remove the fencing, the forklift began rolling forward striking and pinning the employee between the welder frame and the forklift. The employee sustained fractures and a concussion from the impact of the forklift and was hospitalized.
An employee was operating a shear when it came down on the employee's fingers, resulting in amputations to the left index and ring fingers. The guard was in the up position at the time of the incident.
An employee slipped while retrieving a part that had fallen off a suspended conveyor in the parts washing area. The employee's right foot was submerged into a phosphoric acid cleaning solution resulting in burns to his right ankle and 1/2 way up his calf.
An employee was using the "button machine" to crimp pieces of fabric around buttons. One of the pieces of fabric was off center and the employee reached under the guard to slide the fabric back into place. The employee pressed the foot pedal that operates the machine while her hand was still in the machine and amputated the right index fingertip and fractured the right middle finger.
An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.
An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.
