An employee was patrolling a store using an electric patrol scooter when they lost their balance and sustained an ankle fracture.

An employee was operating a forklift to move panels around the dock area when the vehicle struck a joint where the two floors meet. The employee lurched forward and struck his head on the steering wheel. The employee then fell from the vehicle and sustained a dislocated disc in their back, requiring hospitalization.

An employee was standing inside a semi-trailer when the driver of the semi-trailer pulled away, causing the employee to fall backward and strike their head on a steel dock plate. The employee suffered a skull fracture, a fractured L1 vertebra, a broken coccyx, a laceration/abrasion to the head, and a brain bleed.

An employee was riding an ATV, catching up with a stray cow to return it to a pen. The ATV tire struck a hole or other obstacle, causing the employee to be ejected and fall to the ground. His leg was broken and he was hospitalized.

An employee was driving a van in reverse when they fell out of the vehicle and landed on the ground in the parking lot. The employee sustained a fractured left hip.

An employee was operating an electric standing pallet jack and pulling an empty A-frame when the jack struck a guard rail. The employee fell from the lift to his right hip and elbow on the concrete floor, and was hospitalized with a fractured hip.

An employee was clearing yarn that had wrapped around a gear-driven conveyor belt when his left arm got pulled between the belt and a roller. The employee sustained a fractured arm, as well as lacerations and abrasions.

On November 5, 2024, an employee was working on a lamination line. He had removed a slot assembly from a coating station, placed it into a portable slot cleaning tank, and rolled the cart with the assembly out of the cleaning tank. He hit the door of the coating station, and the slot assembly fell from the cart and crushed his little finger. He suffered an amputation above the first knuckle.

An employee was working on the thermal bond production line when the laydown formation table became filled with fabric. The employee was cleaning the area when the machine restarted. The employee's hands were caught between a belt and a roller resulting in amputations to multiple fingers on both hands.

An employee was correcting a malfunction on a covering machine when their left hand was caught in a moving component and they sustained a left index fingertip amputation.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
