The wiring for an argon oxygen decarburization (AOD) motor caught fire and the AOD had to be operated manually. An employee attempted to insert a crank handle into the AOD crank. As the employee was manually tilting the AOD, it moved beyond the center of gravity and the crank handle started spinning rapidly. When the employee tried to grab the crank handle, his right middle finger was caught in the chain and sprocket, de-gloving the fingertip. The chain and sprocket was unguarded at the time of the incident.

An employee was using a jib crane to lower a mold onto a carrier that had a bolt laying on top of it. When the employee attempted to pull the bolt from beneath the mold, it dropped down and smashed his left middle finger between the mold and carrier. His left middle fingertip was partially amputated.

On 4/26/16, at approximately 5:15 AM, an employee was using a pneumatic chipping gun to break up hardened mortar inside the mortar mixer. When he connected his chipping gun's quick-connect hose to the airline, he accidentally hooked it to an oxygen line, which was not properly labeled. While he was operating the chipping gun with oxygen instead of compressed air, a fire broke out inside the tool and enveloped his right arm, face, and neck. He suffered second and third degree burns to his arm, and first and second degree burns to his face and neck, requiring hospitalization.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.

An employee was passing through a large metal door when the door closed on the employee, resulting in a cracked rib.

An employee was preparing to cut lumber on a panel saw when the clamp engaged and caught the employee's left middle finger, resulting in a fingertip amputation.

An employee was standing at the rear of a machine and the machine's mesh tray fell down onto the rear of the machine, crushing his left hand against the read lift. The employee sustained fractures and a laceration to their left hand and was hospitalized.

The injured employee was assisting with tightening down 3/4" hardware during assembly of a 6'x20' triple deck screen. Another employee was using a 1/2" cordless impact wrench to tighten down the hardware. The injured employee was using a 1-1/8" open-end wrench that slipped when the bolt was being tighten down by the impact wrench. The injured employee's right ring fingertip was caught between the open-end wrench and the side sheet of the triple deck screen. The tuft of the distal phalanx was amputated.

An employee was welding baffle plates onto a 55-gallon hydraulic tank. A flash occurred as they began to tack weld a part. The employee suffered burns to most of their face and second-degree burns on each arm, resulting in hospitalization.

An employee was cutting a piece of rubber with a utility knife. The knife slipped and partially amputated the employee's right index fingertip.

On July 22, 2024, an employee was using a pressure washer to fill a mop bucket with water. The washer's hose split and soapy water was injected into the employee's right palm and index finger. The puncture wound had to be surgically cleaned.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
