While performing a medical examination on a patient, an employee stood up and struck their head on a hanging cabinet, resulting in paresthesia related to a head injury. The employee was hospitalized.

On November 3, 2020, an employee was performing a mammogram on a patient. The patient went limp and passed out while clamped in the mammogram machine. When compression of the machine was released, the employee helped guide the patient's fall to the ground. The employee landed on the floor with the patient on top of them and suffered a fractured left ankle and tibia.

On August 26, 2019, at approximately 5:40 a.m., an employee was walking down the hall when she lost her balance, tripped, and fell into the wall. She fractured her left hip and was hospitalized.

An employee was helping to restrain a patient when they tore a pectoral muscle.

An employee was attempting to separate two individuals that were arguing when the employee fell to the floor. The other individuals fell on top of the employee, resulting in a leg injury.

A law enforcement agent was pursuing a suspect on foot when they suffered a head-on collision with the suspect, resulting in a brain bleed and a concussion that required hospitalization.

An employee was holding an upset child and attempted to bring the child to the ground when they rolled their ankle and sustained a fracture in two places. The employee was hospitalized and had surgery.

An employee was knocked down while trying to break up a fight between inmates. His head and upper body struck the metal seat of a dining table before he landed on the concrete floor. He suffered head trauma and a shoulder injury.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
