An employee was on top of a vacuum box, loosening a cap on a vent pipe with a pipe wrench, when he fell approximately 4 feet and landed in a sitting position. He suffered a compression fracture and was hospitalized.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

Lumin8 Transportation Technologies, LLC

An employee was cleaning mud out of the tracks of a mini excavator while the operator had the machine tilted up off the ground to one side. The tracks moved and the employee's left wrist was caught in the gears. The employee sustained puncture wounds to the wrist and two fractures.

An employee was loading a truck with a forklift. The truck drove away from the dock and the forklift fell between the dock and the truck. The employee was hospitalized with injuries to their head, back, and leg.

An employee was moving trucks in the yard. He was working between trucks when a back door swung open and caught their right hand against the frame of another truck. The employee sustained a crushing injury to a finger.

On August 21, 2025, a product development engineer was test riding an all-terrain vehicle (ATV) on a test track when the ATV flipped over. The employee sustained a fractured clavicle and three fractured ribs.

The employee stepped up from the yard/parking lot onto a patio with an approximate 6" rise at the exterior corner of the main office. After his second foot was planted on the patio surface, he lost his balance and fell backward to the concrete ground. The employee was hospitalized with a broken hip that required surgery and a laceration to the elbow.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
