An employee was conducting maintenance activities on the recycle material system of a lamination machine. The employee's right hand was caught in a butterfly valve when the valve closed. The employee sustained an avulsion and partial amputation of the index finger and an avulsion and fracture of the middle finger. The equipment was not locked out at the time.

On February 28, 2023, at approximately 2:00 PM, an employee was stacking product on the boxing line. Her gloves became caught on the chain on the underside of the line, pulling the employee's glove and finger into the sprocket. The employee sustained a partial amputation to the right ring finger.

At 1:19 p.m. on April 28, 2021, an employee was applying tape to a roll on a winder when the tape caught the employee's jacket sleeve. The employee was dragged a quarter-turn around the roll and suffered two broken left-side ribs, a broken right tibia, and a punctured lung.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members.
