An employee was repairing a leaking 40-pound pipe when it fell on his right foot, lacerating and fracturing it. He was hospitalized overnight and had surgery.
An employee had been cleaning an area. He was descending a ladder when he fell to the ground from about halfway up the ladder. The employee sustained fractures to his back and leg.
An employee was opening and adjusting the gate flow using a ratchet to clear the dust in the gate. When trying to clear out the dust, the ratchet went into the gate area and got caught on a conveyor. The employee's finger was pinched between the ratchet and the expanded metal grating, resulting in amputation.
