On October 12, 2023, an employee was cutting a piece of ductile pipe with a demo saw when the saw kicked back and struck the employee below the neck, resulting in a laceration.

An employee was walking to his car when a vehicle struck him. He suffered a fractured pelvis, fractures to three lower-back vertebrae, a broken nose, a head contusion, a laceration to the leg, and a hematoma in the pelvis.

An employee was inspecting a bridge repair project. A gas-powered, walk-behind pavement saw, being used 12 feet away from the employee lunged forward, cutting his left foot. The employee's fourth toe was amputated and he was hospitalized.

An employee had been reviewing an insulated glass unit. The employee was placing the glass unit back in its box when it slipped out of the box and a chipped corner lacerated the employee's left hand.

An employee was on a truck while using a clamp to secure a quartz slab to an A-frame on the truck. A gust of wind pushed the slab onto the employee, resulting in a dislocated right shoulder and wrist.

An employee was shearing off two inches of metal when the metal kicked up and struck his hand. The employee's left ring and middle fingertips were amputated.

An employee was working on a 35,000-pound I.D. fan. As the employee rotated the blades with his foot, the blades continued to spin, lacerating his right foot and amputating a toe.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee tripped over a concrete curb stop in a parking lot. The employee fell to the ground, landing on their left hip and breaking it.

An employee was conducting routine maintenance on a mixer. New lid pins had been installed, and the employee was rotating by hand to ensure they had been installed correctly. The employee's left middle finger was caught between the lid and top of a pin. The momentum from the mixer continued, causing a crushing injury to the finger. The employee underwent a medical amputation from the top knuckle to the tip of the finger.

An employee was climbing down a ladder when it slipped. The employee fell, suffering a broken scapula and broken ribs. The employee was hospitalized.

An employee was climbing down a 15-foot multipurpose ladder. The employee fell about 8 feet, landing on the concrete floor and the ladder itself. The employee suffered several injuries, all on the right side: broken ribs, a broken sternum, a broken temporal bone, broken shoulder, multiple fractures to the cheekbone, and a brain bleed.

An employee was rolling up the landing gear on his trailer when the handle began to unwind and struck his face, fracturing his jaw. The employee was hospitalized and required surgery.
