Final narrative
An employee was cleaning a mill machine when its rotating cutter stack cut his right thumbnail and his right index and middle fingers. The machine was not locked or tagged out at the time.
On August 20, 2025, an employee was adjusting the pneumatic saw stop on a saw in order to cut a piece of aluminum lineal to length. When she flipped the switch to secure the saw stop, her right middle fingertip was caught between the saw stop and the run-off backstop. The employee sustained a fractured fingertip and required surgery.
An employee was changing the die on an extrusion press machine when the machine actuated amputated the employee's right index finger to the first knuckle. The die was energized at the time of the incident.
