On March 16, 2023, an employee was utilizing a forklift outside of the facility to clean up trash when the vehicle became stuck. The employee then went into the building to use another forklift to pull the stuck forklift out with a chain/hook. While pulling out the stuck forklift, the chain and hook broke, striking the employee in the head/face. The employee sustained injuries to the head that included a fractured skull, brain bleed, the loss of seven teeth, and lacerations. The employee was hospitalized.

An employee was flipping a cylinder when their hand was caught between the cylinder and the floor resulting in a left index fingertip amputation.

An employee was sliding a gear box cover off a pallet by hand when his left middle finger was caught between the gear box cover and the wooden pallet, resulting in a finger amputation.

An employee was operating a crane with a sling to lift a cylinder from a basket when the employee's left ring finger was caught between the sling and the cylinder, crushing it and resulting in an amputation to the first joint.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

On March 26, 2025, an employee was standing on a work table that was 23" high to abrasive blast the upper section of a pump. When he stepped off the table with his left leg, his left knee gave out and he fell to the ground, landing on his right hip. The employee sustained a displaced right hip fracture near the joint and was hospitalized.

An employee was pressure washing a suspended part when the part started spinning. The employee manually stopped the part from spinning when his hand was caught in the pressurized water blast. The employee was hospitalized with a laceration.

An employee was dumping a "self-dumping chip bin" (on the forks of a forklift) into a 3- or 4-foot-tall dumpster. The bin tilted forward. Thinking it was going to slide off the forks, the employee reached to grab the bottom of the bin to stop it. The bin then rotated back. The tip of the employee's left middle finger, from the first knuckle past his fingernail, was caught between the bin and the top of the forklift pocket frame. The fingertip was amputated just above the first knuckle.

An employee suffered two fingertip amputations while working at a plant.

An employee was operating a stand-up forklift in the warehouse when the forklift spun and the employee was caught between the forklift and metal shelving. The employee sustained multiple broken ribs.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was working in the food service warehouse when he experienced chest pain, difficulty breathing, and lost consciousness. The employee sustained carbon monoxide poisoning.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.

An employee was working at her desk. She went to stand up and fell to the floor beside the desk. The employee sustained a hip displacement and required surgery.

An employee was processing wood boards at a chop saw when the saw malfunctioned and the blade cut her left hand and fingers. She was hospitalized and her little finger was surgically amputated.
