An employee was changing a bearing on an auger. He stepped down on the auger, which was still moving, and his foot was caught between the auger and the wall, amputating his right pinky toe. The auger was not locked/tagged out at the time.
Four employees were dislodging a canister cart ring from a canister. The ring shifted clockwise and amputated the tip of one employee's right index finger.
