A correctional officer was attempting to secure an inmate who had a suspicious item. He was holding onto the inmate with his feet planted on the ground as the inmate tried to break away from him. This caused the officer to fall to the ground. He was hospitalized with dislocated kneecaps.

An employee slipped on an icy walkway and fell, hitting his head on the ice.

On 12/23/2017, at approximately 7:30 AM, an employee was attempting to plug a grill in when he contacted the plug and received an electrical shock.

An employee was operating a band saw when his right little fingertip was amputated.

An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.

An employee was operating a trim saw. While trying to free a jammed board she had just cut, she came into contact with the running saw blade, which amputated her right thumb.

An employee was using a shaper to cut wood when their right hand contacted the blade, resulting in a partial amputation to the right ring finger.

An employee was working in the food service warehouse when he experienced chest pain, difficulty breathing, and lost consciousness. The employee sustained carbon monoxide poisoning.

An employee was responding to a physical altercation between two inmates when they slipped and fell on the ground, landing on their left arm. The employee was hospitalized with a compound fracture to their left forearm.

An employee was working on an out-of-service forklift when his hand was caught in the door frame as the stuck cab door was being opened. The employee sustained a middle fingertip amputation.

An employee was closing a door while holding the door handle behind him. The door closed on his finger resulting in a fingertip amputation.

An employee placed his hand down and was shocked by electricity from a misting fan.

An employee delivered a package. On her way back to her vehicle she was attacked by two dogs that came from around a corner. She was hospitalized with dog bites.

The injured employee was standing by the controls for the electric spud system on the barge, preparing to raise the spuds. Meanwhile, an excavator was moving a mat on the deck. As the excavator swung from port to starboard, it's counterweight struck the injured employee s shoulder, causing them to lose balance and fall against the spud controls. The employee sustained bruises and contusions to their right shoulder and chest.

A technician was operating a crane hoist to lift/remove a cylinder head from the main engine (port). The load was lifted at an angle, causing the cylinder head to get stuck on the cylinder head studs. When it became free from the studs, the cylinder head swung. It struck the technician and caught them against the cabinet door. This resulted in an amputation of their left index fingertip at the distal knuckle.

An employee was securing a power cord to a refrigerated truck container. A truck started backing up to connect with the container and the employee was pinned between the tires of the container and the bolster of the truck, resulting in a fractured leg.

An employee was delivering a package to a house when a dog came through the storm door, jumped on the employee, and bit them in the lower jaw.
