The employee was walking and stepped on a bin top cover which shifted, causing the employee to fall forward and strike his face on the tripper rail and a piece of conduit. The employee sustained a fractured eye socket/cheek bone.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was helping clean the bag house. He checked under the auger in the dust collection system to feel for airflow. His right hand was sucked up into the auger blade, resulting in amputation of the middle finger to the distal joint. Surgery was required to amputate to the finger to the second knuckle.
