Final narrative
On September 8, 2017, an employee was helping to dismantle a scaffold when he stepped backward and fell approximately 6 feet to the ground. He fractured his right ankle and was hospitalized.
An employee was standing on stilts installing acoustic ceiling grids. He saw a ladder leaned on a corner and a 3-foot diameter plastic ring on the ground. When he took a step, one of the stilts got caught against the ring throwing the employee off balance. As he attempted to regain balance with his other foot, the employee stepped on a piece of conduit lying on the ground causing him to slip and fall. The employee sustained a right femur fracture.
An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car.
