An employee was walking on a sidewalk, heading toward her car, when she tripped and fell onto her left hip. The employee sustained a left intertrochanteric fracture requiring hospitalization and surgery.

An employee was rising from a chair when their foot caught on the chair, causing them to fall to the floor. The employee was hospitalized with a displaced fracture of the right humerus.

An employee was leaving a patient's home when they slipped on ice going down the steps and sustained a fractured ankle.

An employee was working as a security guard. A trespasser had entered the hospital and was confronted by the police. A physical altercation occurred and the trespasser was inadvertently pushed into the employee causing them to fall to the ground and strike their head on the ground. The employee sustained a subarachnoid hemorrhage.

An employee moving a patient felt throbbing pains in her lower back and was hospitalized with a back sprain.

Employee 1 was operating a tow tractor and suffered a medical event. Employee 2 came to help and employee 1 fell on employee 2. This caused employee 2 to fall and sustain a broken hip.

A security officer was helping to restrain a patient and suffered a laceration to the right hand and a bloody nose. The right hand became infected and required surgical debridement.

A field service technician was servicing a lottery machine behind a counter. An altercation began between a customer and another employee, during which the technician suffered a broken ankle.

An employee was trying to resolve an altercation between two patients when a patient hit him and injured his left eye, resulting in eye surgery.

On May 20, 2020, at approximately 7:15 PM, an employee was removing an aggressive client to a seclusion room when the client kicked the seclusion room door. The door slammed on the employee's finger, resulting in a partial amputation of her right middle fingertip.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee slipped on water in a hospital hallway, fell, and landed on his left hip. He suffered a contusion and fracture to the hip.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
